In-Person Visits

IN-PERSON APPOINTMENTS

We require that your initial visit be in-person at our clinic in West Des Moines. Follow-up appointments can be either in-person or virtual.  


Please bring:

  1. a form of ID (driver's license)
  2. your current insurance card
  3. a form of payment (debit card, credit card, FSA, HSA, cash, check)
  4. a list of your medications and their dosages


When you check in for your visit, please notify us if there have been any changes to your address, phone number, email address, or legal name. 


Please notify us as soon as possible BEFORE your appointment if there have been any changes to your insurance plan/coverage.

We are located off of Westown Pkwy near Valley West Mall. The building can be accessed by turning onto either 29th St or 30th St.

Enter the front doors of the building (facing the street) and take the elevator or stairs to the 2nd floor. Turn down the hall to the left and Focused Life Clinic is the first suite on the left.
