Insurance Information

In-network typically indicates that there is an agreement between a clinic and the insurance to provide care at a negotiated price. This means that this is most likely the most cost effective option for you as the patient. While we have agreements with the following insurance groups/insurance plans, it does not guarantee that the provider is covered or that the service is covered. To prevent unexpected cost, we recommend calling your insurance company and make sure that the provider you are scheduled to see is covered and that you have active benefits.

Please notify us as soon as possible BEFORE your appointment if there have been any changes to your insurance plan/coverage. 

Some insurance polices may be based out of a different state or the provider may not have an agreement with a specific insurance group or policy. In this case your insurance may charge you more to see your preferred provider. In these cases, we recommend discussing paying out of pocket for your services as this may be a more cost-effective way of seeing the provider that you would like. We are also able to ask the insurance for individual contracts as an alternative option. If your insurance says that we are not in-network, please notify us and we are happy to see how we can assist in making sure that you receive the treatment that you need.

• Medicaid
• Tricare

Current rates can be found below for patients that have financial need or are uninsured.

• Established Patients - 
  • Follow-Up (30 minutes) - $150 (needs-based pricing adjustment available)
  • Follow-Up (50 minutes) - $230 (needs-based pricing adjustment available)
